This is the form and self-service administration area for our volunteers.  You will find forms, and instructions on obtaining a police check and a WWCC.  We thank you for being a part of working to empower individuals with Down syndrome for a lifetime of meaningful inclusion in the community.

Working With Children Check Procedures

Our volunteers require a Working with Children Check. Application forms can be obtained from participating Australia Post outlets.

Steps to completing a form:

1. Fill out the enclosed Working with Children Check as per the attached information in the form itself.

2. For section D, refer to the back page of the form. Please note that codes specific to volunteering with Down Syndrome Victoria are: 

    (10), (74) and any others that may be relevant to you, eg (34) for photography services
 
3. For section E, enter Down Syndrome Victoria’s details:

4. Do not sign the form, or attach your photo, as you will need to do so when you lodge your application at an Australian Post Office.

If you require a Working with Children Check for employment purposes as well as volunteering, please note that you will have to fund the check yourself. The check can then be used for both your employment needs and volunteering with Down Syndrome Victoria.

Police Check Procedures

Several of our volunteer positions require a police check.  Down Syndrome Victoria will assess if a Police Check is required (based on the tasks you will be doing).  You will be notified if we require a Police Check from you, and we will be guide you through the steps involved.
